PURPOSE OF THE UNIT STANDARD 
This unit standard is for persons required to render care and manage clients and significant others at home and to provide relevant information to clients and significant others. The learner will have a fundamental knowledge of the diseases common to the learners' working environment.

The qualifying learner will be capable of:
  • Providing information relating to disease and disability to individuals and groups.
  • Assessing the health status of the client.
  • Assisting in the management of the client's daily living activities.
  • Assisting in the maintenance of the client's health status.
  • Providing psycho-social support to the client and significant others.
  • Assessing and taking responsibility for personal well-being. 

  • LEARNING ASSUMED TO BE IN PLACE AND RECOGNITION OF PRIOR LEARNING 
    Communication at NQF Level 2. 

    UNIT STANDARD RANGE 
    The care will take place under the direct and indirect mentorship and supervision of a professional with relevant health care knowledge and experience.

    This unit standard applies to persons rendering care and assistance within the home environment. Home may be defined in a variety of ways, e.g. all types of care including community care institutions.

    SPECIFIC OUTCOME 1 
    Provide information relating to disease and disability to individuals and groups.

    ASSESSMENT CRITERION 1 
    Information provided is relevant to the needs of the client and the significant others and is presented in a manner that is appropriate to their level of understanding. 

    ASSESSMENT CRITERION 2 
    Information is presented to individuals or groups in the community in a culturally appropriate manner. 

    ASSESSMENT CRITERION 3 
    Disease patterns, conditions and disability issues in the local community are described in terms of the impact on health status. 

    SPECIFIC OUTCOME 2 
    Assess the health status of the client.

    ASSESSMENT CRITERION 1 
    The current health condition of the client is determined through the use of suitable assessment skills and accredited tools. 

    ASSESSMENT CRITERION 2 
    Communicable and chronic diseases and conditions are identified in terms of their signs, symptoms and treatment. 

    ASSESSMENT CRITERION 3 
    Congenital and hereditary diseases are identified and assessed in terms of their impact on the health status of the client and family. 

    ASSESSMENT CRITERION 4 
    Significant others coping skills are assessed in terms of the resources and ability to care for the client.

    Assist in the management of the client's daily living activities.

    The client's daily activities are identified and assessed in terms of potential problem areas. 

    ASSESSMENT CRITERION 2 
    Problem areas identified are explained to the client and significant others, and remedial action suggested is suitable to the specific situation. 

    ASSESSMENT CRITERION 3 
    All equipment used in the performance of activities is relevant to the needs of the client and is maintained and used correctly and the client and significant others are instructed in the use and maintenance of equipment where applicable. 

    ASSESSMENT CRITERION 4 
    An activities of daily living schedule is compiled in consultation with the significant others and the importance of adherence to and maintenance of the care plan is emphasised. 

    ASSESSMENT CRITERION 5 
    Interventions are based on the principles of body mechanics and relevant safety measures.

    Assist in the maintenance of the client's health status.

    ASSESSMENT CRITERION 1 
    All practical care procedures performed are relevant to the needs of the client and are carried out in accordance with accepted nursing care principles and relevant scope of practice. 

    ASSESSMENT CRITERION 2 
    The client and significant others are instructed in ways of maintaining health status, restoring function and preventing complications. 

    ASSESSMENT CRITERION 3 
    Waste matter is disposed of in accordance with hygiene and safety principles and in terms of available services. 

    ASSESSMENT CRITERION 4 
    The client and significant others are instructed in the accepted waste disposal procedures to maintain hygiene and safety and prevent the spread of disease.

    Provide psychosocial support and refer when necessary.

    Counselling is provided to the client and significant others to assist them in dealing with the situation.

    Methods of accessing support are explained to family members in terms of referrals and accessing specific benefits.

    The ability to take responsibility for the maintenance of self-health when working with clients is demonstrated in terms of recognizing potential need of support and accessing it.

    Own feelings and experiences relating to care situations are identified in terms of monitoring and managing own coping levels. 

    ASSESSMENT CRITERION 3 
    Active participation in group support systems is ensured on a formal and informal basis. 

    ASSESSMENT CRITERION 4 
    Active anticipation in the formation and maintenance of local networks is ensured on a formal and informal basis.
